HIRING A GRADUATE ASSISTANT
               
                  - Prepare the letter of offer located on

                        - Required: all Graduate Assistants.
 
- Disclosure and consent form for criminal background (

                        - Required: All students who have not held a graduate assistant position in the six months prior to this appointment.
- Attach completed form to appointment/reappointment form in the EBS portal.
 
- Employment Eligibility Verification Form I-9 (

                        - Required: All new hires to the university or those students who have not worked in some capacity in the 12-months prior to this appointment.
 
- CPT/OPT Form – Completed by student and OISS
                        - Required: International Graduate Assistants working more than 50% during Fall and/or Spring semesters.
- Attach completed form to appointment form in the EBS portal.
 
- Appointment/Reappointment form
                        - Required: all Graduate Assistants.
- Initiate through the EBS portal.
 
- Graduate Assistant Residency Exception Form – Activates tuition waiver and health insurance.
                        - Required: all Graduate Assistants.

- Parking information
                        - Required: all new Graduate Assistants requiring a parking permit before GA appointment has been approved in the EBS portal.
- Prepare a letter for the GA to provide to the parking office to obtain a parking permit.
